Notes:
The following information is required to receive services:
- A driver's license or picture identification card from the head of household and all adult members of the household.
- The address on the ID must be a current address in Perquimans County or an alternate proof of address is required such as a utility bill, bank statement, or government mail.
- Your food stamp card if you have one.
- If not receiving food stamps, a list of income and expenses will be required, as well as a picture ID for adults and birth certificate or social security card for each child.
